Relevance. The increasing prevalence of early-onset myopia highlights the need to identify biometric indicators of refractive disorders at the preschool age. Objective. To determine the biometric characteristics of the eyes in children aged 4–7 years with varying degrees of visual impairment based on a two-stage ophthalmological examination. Material and methods. At the first stage, 1,284 children were screened; reduced or borderline visual acuity was found in 25.9% of them. At the second stage, 408 children underwent a comprehensive examination including cycloplegic autorefractometry, ultrasound biometry (AL, CR, AL/CR), assessment of accommodative reserve, and analysis of environmental visual factors. Results and conclusion. Children with pronounced visual impairment demonstrated a significant increase in axial length (23.06 mm), a higher prevalence of AL/CR > 3.0 (38.6%), and reduced accommodative reserve, indicating early signs of myopization. Category A showed intermediate characteristics, while the control group remained within age-appropriate norms. The two-stage examination model proved highly informative and can be recommended for early identification of atrisk children and for developing targeted preventive strategies.
