Relevance. Early detection of refractive errors in preschool children remains insufficient. Despite reduced visual acuity in a considerable proportion of children, the rate of ophthalmologic consultation is extremely low. Purpose of the study. To assess the prognostic value of clinical, biometric, and behavioral risk factors for the development of refractive errors in preschool children. Material and Methods. A total of 1,284 children aged 5–7 years were examined. At the second stage, 408 children underwent comprehensive ophthalmic evaluation, including assessment of spherical equivalent, axial length, and AL/CR ratio, along with behavioral and hereditary factors. Risk stratification was performed. Results and Conclusion. Reduced visual acuity was detected in 15.7% of children, while only 0.7% had previously consulted an ophthalmologist. Increasing risk level was associated with a decrease in spherical equivalent (from +1.50 ± 0.70 to 0.00 ± 0.50 D), an increase in axial length (from 22.09 ± 0.55 to 23.19 ± 0.51 mm), and AL/CR ratio (from 2.85 ± 0.06 to 3.05 ± 0.05), as well as changes in visual load parameters. After 24 months, the proportion of high-risk children decreased from 30.5% to 17.7% in the main group, whereas it increased to 35.1% in the control group. Combined assessment of biometric parameters and visual load factors enables early identification of children at high risk of myopia at the preclinical stage.
