Relevance. The constant use of glaucoma drugs is the basis of the drug treatment of glaucoma. However, long-term use of eye drops containing preservatives, especially benzalkonium chloride, negatively affects the condition of the eye surface and contributes to the formation of iatrogenic dry eye syndrome (DED). Purpose of the study. To evaluate the therapeutic efficacy of Gilan® lacrimal substitute containing sodium hyaluronate and trehalose in patients with iatrogenic dry eye syndrome caused by prolonged use of glaucoma drops. Materials and methods. The study included 35 patients (70 eyes) with glaucoma of various stages, in which intraocular pressure was compensated when using topical glaucoma drugs containing benzalkonium chloride as a preservative. The duration of therapy ranged from 3 months to 14 years. In addition to antihypertensive therapy, all patients were prescribed Gilan ® Extra. Ocular surface assessment was performed prior to initiation of lacrimal replacement therapy and at 1 and 6 months after administration. The efficacy of therapy was determined using biomicroscopy, functional tests (Norn, Schirmer and Jones samples). Results and conclusion. Against the background of combined use of antihypertensive drugs and a tear substitute containing sodium hyaluronate and trehalose, 88% of patients showed an improvement in the state of the corneal epithelium. The mean rupture time of the precorneal tear film significantly increased: after 1 month - more than 1,6 times, after 6 months - 2,1 times (p <0.001). Total and primary tear production increased by 3.7% and 17.6% (p = 0.039) and by 8.6% and 19.3% (p = 0.068), respectively.
