This study presents a forensic histological analysis of brain tissue obtained from 43 patients who died as a result of traumatic brain injury. Histomorphological examination revealed a high frequency of perivascular edema (90.7%), venous congestion (86.0%), and neuronal dystrophy (83.7%). These alterations were closely associated with ischemic hypoxia and disruption of the blood–brain barrier, serving as the primary morphological causes of death. In cases of vehicular trauma, acute ischemic shock and cerebral edema predominated, while falls from height were characterized by arteriolar spasm and microhemorrhages. Forensic histological assessment confirmed that ischemic and vascular changes in the brain can serve as reliable morphological markers of death. The results emphasize the diagnostic value of histomorphological evaluation in determining the cause and mechanism of death in forensic medical practice.
