Tuberculosis (TB) remains a leading cause of infectious disease mortality worldwide, with children representing a population at heightened epidemiological risk. This prospective controlled study evaluated the diagnostic and prognostic significance of key immunological parameters in 84 pediatric patients aged 3–14 years: 42 with confirmed active pulmonary tuberculosis and 42 matched healthy controls. T-lymphocyte subpopulations (CD3+, CD4+, CD8+) were assessed by flow cytometry, and serum IFN-γ and IL-4 levels were quantified by ELISA. Active TB was associated with marked suppression of CD4+ T-helper lymphocytes, inversion of the immunoregulatory index (IRI), and profound Th1/Th2 cytokine dysregulation (elevated IL-4 and reduced IFN-γ). These findings substantiate the clinical utility of the CD4+/CD8+ ratio and IFN-γ/IL-4 balance as quantitative biomarkers for disease severity stratification and treatment monitoring in pediatric tuberculosis.
