Hyperandrogenism is a common endocrine disorder in women of reproductive age and a major contributor to reproductive dysfunction. Excess androgen production may originate from ovarian sources, predominantly in polycystic ovary syndrome, or from adrenal origins, particularly in non-classical congenital adrenal hyperplasia. Persistent hyperandrogenism disrupts hypothalamic-pituitary-ovarian axis regulation, impairs folliculogenesis, and leads to chronic anovulation. Metabolic disturbances, especially insulin resistance, enhance steroidogenesis and decrease sex hormone-binding globulin levels, increasing circulating bioactive androgens. Hyperandrogenism is associated with infertility, impaired endometrial receptivity, and adverse pregnancy outcomes. Current evidence on the distinctions and interrelationships between ovarian and adrenal forms is summarized.
