This work is devoted to the study of the role of macrophages (designated by the CD68 marker) in the immune microenvironment of soft tissue sarcomas and their impact on the effectiveness of therapy. The analysis includes an assessment of the localization of macrophages inside and around the tumor, as well as their relationship with morphological and clinical parameters of treatment responses. The results showed that the presence of macrophages inside the tumor tissue is significantly associated with a more pronounced morphological regression and a high level of therapeutic pathomorphosis, which indicates an active antitumor role of macrophages with the M1 phenotype. At the same time, it was found that the CD68 status is not a predictor of short-term clinical effects, such as a complete or partial response. The study emphasizes the importance of assessing the functional activity of macrophages and their phenotypic status for a more accurate prognosis and personalization of therapy for soft tissue sarcomas. The data obtained contribute to an expanded understanding of the immune mechanisms involved in the regression of tumor structures during treatment and can serve as a basis for the development of new immunotherapy strategies.
