Community-acquired pneumonia remains one of the leading causes of morbidity and mortality among children worldwide, while antimicrobial resistance poses an increasing threat to successful clinical treatment. The emergence and rapid spread of antibiotic-resistant bacterial strains have significantly reduced the therapeutic options available to physicians treating pediatric pneumonia, necessitating the exploration of adjunctive treatment strategies capable of improving clinical recovery and restoring microbial balance. This study was conducted to evaluate the clinical efficacy, safety profile, and microbiome-modulating effects of the postbiotic preparation Biodonatum when used as an adjunct to standard antibiotic therapy in children diagnosed with community-acquired pneumonia and exhibiting reduced susceptibility to first-line antibiotics. A prospective, randomized, controlled clinical trial was conducted at the Department of Pediatric Infectious Diseases of the Tashkent State Medical University. The study enrolled 90 children aged 3 to 14 years with confirmed community-acquired pneumonia and documented reduced sensitivity to antibiotics. Participants were randomly assigned to two equal groups: the intervention group (n = 45) received standard antibiotic therapy supplemented with Biodonatum, while the control group (n = 45) received standard antibiotic therapy alone. Clinical, laboratory, and microbiological parameters were assessed at baseline and on days 7 and 14 of treatment.
