Proximal humerus fractures (PHFs) are among the common injuries of the musculoskeletal system, especially in older age groups. In young patients, they are more often associated with a high-energy mechanism and may be accompanied by complex morphology, dislocation, soft tissue injury, and damage to vascular and nervous structures. Military personnel represent a special category, since the mechanism and circumstances of injury require an extended assessment of associated injuries. The article analyzes modern Neer and AO/OTA classifications, as well as the capabilities of radiography, computed tomography, 3D reconstruction, magnetic resonance imaging, and vascular imaging. It is shown that classification systems have limited interobserver reproducibility, while CT is most useful in complex fractures and preoperative planning. The need for a comprehensive diagnostic algorithm is emphasized, taking into account the mechanism of injury, fracture morphology, the condition of the medial support, soft tissues, and neurovascular status.
