Annotatsiya

In recent years, severe destructive lesions of the upper and lower jaw bones associated with the use of homemade injectable psychoactive substances, including desomorphine, various types of methamphetamine, and other substances, have been increasingly reported in medical practice. These lesions include toxic osteonecrosis and concomitant odontogenic osteomyelitis. Based on a literature review, this article systematizes the main pathogenetic mechanisms of this pathology: the accumulation of toxic impurities, primarily red phosphorus, in the bone matrix; impaired microcirculation and endothelial function; delayed reparative processes due to reduced osteoclastic activity; and the development of odontogenic infection against the background of secondary immune deficiency. The obtained data provide a theoretical basis for improving pathogenetically grounded diagnostic and treatment strategies.
