The authors report that the MiLEP technique, with postoperative laser therapy and a reduced instrument diameter (19 Fr), decreases the incidence of blood loss, urethral injuries, and infectious complications in the early postoperative period. In the long term, there was nearly a fivefold reduction in the frequency of urethral strictures, a threefold decrease in retrograde ejaculation, and complete absence of repeat interventions. IPSS, Qmax, and QoL scores demonstrated sustained improvement. The authors conclude that the optimized surgical approach enhances the radicality of the procedure, improves recovery of urodynamic functions, and elevates the quality of life of patients, especially among older individuals with comorbid conditions.
