This study evaluated the association between CYP2E1 c2 allele carriage and the clinical and laboratory severity of alcohol-related liver cirrhosis. Sixty patients were divided into a c1/c1 genotype group (n=39) and a c2-carrier group with c1/c2+c2/c2 genotypes (n=21). The duration of alcohol use was longer among c2 carriers: 12.9±1.2 versus 10.8±1.0 years. They also had higher AST, AST, ALT ratio, total bilirubin, and INR values and lower serum albumin. The mean MELD, MELD-Na score was 20.2±5.2 versus 15.4±4.3 points (p<0.001). Child-Pugh class C was identified in 42.9% of c2 carriers and 17.9% of patients with the c1/c1 genotype; c2 carriage was associated with 3.43-fold higher odds of severe functional class (95% CI: 1.04-11.27; p=0.037). These findings indicate that c2 carriage is associated with more pronounced cytolysis, impaired hepatic synthetic function, and a more severe clinical phenotype of alcohol-related cirrhosis. However, the longer duration of alcohol exposure among c2 carriers should be considered a potential confounding factor.
