This article presents a systematic analysis of contemporary tubeless and totally tubeless percutaneous nephrolithotomy (PCNL) techniques. Based on data from randomized controlled trials and meta-analyses, the clinical outcomes of tubeless PCNL are compared with those of standard nephrostomy drainage. The rates of intraoperative and postoperative complications, pain scores, length of hospital stay, and analgesic requirements are assessed. Evidence indicates that, in carefully selected patients with uncomplicated renal stones, tubeless approaches achieve stone-free rates comparable to those of standard PCNL while significantly reducing postoperative pain, opioid consumption, and length of hospitalization. Patient selection criteria, contraindications, and prospects for widespread adoption in routine urological practice are also discussed.
