Relevance. Glaucomatous optic neuropathy (GON) is one of the leading causes of irreversible blindness and is associated with neurodegenerative, vascular, and metabolic disturbances. Modern optical coherence tomography techniques enable objective assessment of structural and microcirculatory changes; however, the role of metabolic biomarkers in monitoring neuroprotective therapy remains insufficiently studied. Purpose of the study. To evaluate the relationship between structural, vascular, and metabolic biomarkers of GON based on correlation analysis of OCT/OCTA findings and nicotinamide and arginine levels in order to determine their diagnostic significance. Materials and methods. The study included patients aged 40-75 years with early and moderate primary open-angle glaucoma. Structural OCT and neurometabolomic analysis of blood plasma and tear fluid using liquid chromatography–tandem mass spectrometry (LC-MS/MS) were performed. Retinal nerve fiber layer (RNFL) thickness as well as nicotinamide and arginine concentrations were assessed. Correlation analysis between metabolic and structural-functional parameters was conducted. Results. Patients with GON demonstrated a significant reduction in RNFL thickness and capillary density, as well as decreased nicotinamide and arginine levels, most prominently in tear fluid (p<0,05). Statistically significant positive correlations were found between metabolite levels and RNFL thickness and mean deviation (MD) values (r=0,33-0,44), while negative correlations were observed with the cup- to-disc ratio (r= –0.30 to -0.41). Conclusion. The integration of OCT findings with metabolomic analysis provides a more objective assessment of GON severity and improves the potential for personalized monitoring of neuroprotective therapy.
