Background. Reactive arthritis (ReA) is frequently accompanied by anterior segment involvement and lacrimal gland dysfunction. This issue remains insufficiently studied in the Central Asian population, and in many cases, diagnosis is delayed. Purpose of the study. To characterize the spectrum of anterior segment pathologies in patients diagnosed with ReA, to assess lacrimal secretory function, and to compare these parameters with a healthy control group. Material and Methods. A prospective case-control study was conducted at the Multidisciplinary Clinic of Tashkent Medical University between 2025 and 2026. Thirty patients (60 eyes) with confirmed ReA and anterior segment complaints, along with 20 healthy volunteers (control group, 40 eyes) matched by age and sex, were enrolled. All participants underwent a standard ophthalmological examination including visual acuity testing, biomicroscopy, tonometry, and Schirmer I test. Results and Conclusion. The study group comprised 18 males (60%) and 12 females (40%), with a mean age of 32.5±7.2 years. The most common findings were conjunctival hyperemia (70.0%), anterior uveitis (40.0%), keratitis (25.0%), episcleritis (15.0%), and dry eye syndrome (53.3%). The Schirmer test result in the ReA group (7.8±1.4 mm) was significantly lower than in the control group (15.1±1.7 mm; p<0.001). Reactive arthritis is frequently associated with clinically significant anterior segment inflammation and lacrimal gland dysfunction. Regular ophthalmological screening of ReA patients and close collaboration between rheumatologists and ophthalmologists are essential to prevent visual complications.
