Relevance. Keratoconus remains an important problem in modern ophthalmology, requiring not only effective treatment but also objective assessment of its results using biomechanical and immunological biomarkers. Purpose of the study. To comprehensively evaluate the effectiveness of corneal crosslinking in patients with progressive keratoconus based on the dynamics of Corvis ST biomechanical parameters and tear lactoferrin levels. Materials and methods. Twenty-five patients (35 eyes) with stage I-II keratoconus were examined. Biomechanical parameters (CBI, TBI, DA, SP-A1), topographic indices (Kmax, ISV, IVA), and tear lactoferrin levels were evaluated before and 12 months after crosslinking. Results. After CXL, significant improvement in biomechanical parameters was observed: CBI decreased from 0.68±0.12 to 0.52±0.11 (p<0.001), TBI from 0.85±0.14 to 0.65±0.13 (p<0.001), DA from 1.32±0.18 to 1.18±0.15 mm (p<0.05). Lactoferrin level increased from 0.52±0.28 to 1.21±0.42 mg/ml (p<0.001). Correlation between lactoferrin restoration and improvement in biomechanical parameters was revealed.
