Background. Accurate energy dosimetry remains an open issue in the treatment of central serous chorioretinopathy (CSCR) using 577 nm subthreshold micropulse laser treatment (SMLT). Traditional test coagulation is subjective and carries a risk of photoreceptor damage in the test area. Purpose of the study. To develop an objective, non- invasive laser energy dosimetry method based on the patient’s individual fundus pigmentation and to evaluate its clinical efficacy. Materials and methods. Patients with CSCR were divided into two groups: in the control group, laser energy was selected via traditional continuous-wave titration, while the study group utilized a new algorithm evaluating individual fundus optical density. Outcomes were assessed using OCT, FA, and best-corrected visual acuity (BCVA) dynamics. Results. High dosing accuracy was achieved in the study group without local tissue damage from test burns. Follow-up revealed complete subretinal fluid resolution, visual acuity improvement, and a total absence of iatrogenic photoreceptor damage. Conclusion. The new objective dosimetry method based on fundus pigmentation eliminates the need for hazardous test coagulation during SMLT, minimizes the risk of tissue damage, and significantly enhances clinical and functional treatment outcomes.
