Relevance. Hepatocyte Growth Factor (HGF) is a versatile protein with multiple effects, produced by corneal cells and the lacrimal gland. In experimental settings it promotes the proliferation of the inner corneal cells, strengthens their ability to migrate, and makes them more resistant to oxidative stress. The fact that it can be detected in tear fluid points to a natural regulatory pathway that supports the recovery of the inner corneal layer. Yet how closely the preoperative level of this factor relates to the outcome of keratoplasty has not been fully explored until now. Purpose of the study. To assess how the preoperative tear fluid level of hepatocyte growth factor affects the functional outcomes of DSAEK surgery in patients with Fuchs endothelial corneal dystrophy, and to evaluate the dynamics of its changes over time. Materials and methods. Twenty-four patients with Fuchs endothelial corneal dystrophy were examined. Results. The level of hepatocyte growth factor followed a two-stage course: from a baseline mean of 478 pg/ml it rose to a peak of 870 pg/ml by the third month and then settled at around 815 pg/ml by the sixth month. Patients whose preoperative level was 500 pg/ml or higher showed a marked improvement in visual acuity at six months (0.57±0.04 compared with 0.34±0.05; p=0.003), and the graft also attached more securely in these patients. With the chosen graft delivery method, the loss of cell density was only 8 percent. Conclusion. Measuring the level of hepatocyte growth factor in tear fluid before surgery makes it possible to anticipate how successfully the operation will go and to predict the degree of corneal transparency in advance.
