ДИНАМИКА ИЗМЕНЕНИЙ ГЛАЗНОГО ДНА И ПАРАМЕТРОВ МИКРОЦИРКУЛЯЦИИ ВО ВРЕМЯ ПРИСТУПА МИГРЕНИ (КЛИНИЧЕСКИЙ СЛУЧАЙ)

Relevance. Migraine is among the most prevalent neurological disorders and is considered to result from a complex interaction between neuronal and vascular mechanisms. Despite extensive research on cerebral hemodynamics in migraine, direct visualization of retinal vascular alterations during an acute migraine attack remains insufficiently investigated. Clinical case description. We report the case of a 32-year-old woman with episodic migraine with aura who underwent comprehensive ophthalmological evaluation during the interictal period, during a spontaneous migraine attack, and following complete resolution of clinical symptoms. Funduscopic examination performed during the attack revealed diffuse narrowing of the retinal arterioles accompanied by a reduced arteriovenous ratio. Optical coherence tomography angiography (OCTA) demonstrated a decrease in vessel density within both the superficial and deep capillary plexuses, enlargement of the foveal avascular zone, and a reduction in peripapillary vascular density. Following resolution of the attack, near-complete restoration of all assessed parameters was observed. Conclusion. The present findings provide evidence of reversible retinal microcirculatory disturbances occurring during a migraine attack and support the role of transient vascular dysregulation in the pathophysiology of migraine.
