Relevance. Cataract is highly prevalent among older adults and significantly impairs visual function, quality of life, and daily activity. In addition to lens opacity, microcirculatory disorders of ocular tissues play an important role, reflecting vascular maladaptation. Studying these parameters at different stages of cataract makes it possible to better understand the mechanisms of the disease and justify a comprehensive assessment of the functional state of ocular tissues. Purpose of the study. To study the features of ocular tissue microcirculation in patients with cataract depending on the stage of the disease using the LAZMA-PF device. Materials and methods. The study included 200 individuals aged 50 years and older: 150 patients with cataract and 50 controls without significant ophthalmic pathology. Patients with cataract were divided into three subgroups: incipient, immature, and mature cataract, with 50 patients in each subgroup. All participants underwent standard ophthalmological examination, including visual acuity testing, biomicroscopy, and ophthalmoscopy, as well as assessment of ocular tissue microcirculation using the LAZMA-PF device. Results. Patients with cataract demonstrated stage-dependent deterioration of microcirculatory parameters, manifested by decreased tissue perfusion, increased coefficient of microvascular blood flow variation, and elevated vascular tone. The most pronounced changes were observed in mature cataract, where reduced perfusion was combined with the greatest increase in blood flow variability and vascular tone. Significant correlations were established between cataract stage, decreased visual acuity, and the severity of microcirculatory dysfunction. Conclusion. Patients with cataract have pronounced disorders of ocular tissue microcirculation depending on the stage of the disease, characterized by reduced tissue perfusion, instability of microvascular blood flow, and increased vascular tone.
