Relevance. High myopia may be associated with increased axial eye length, making accurate intraocular lens power calculation challenging. A comparison of the outcomes of paired and single intraocular lens implantation is of practical importance for selecting the optimal surgical strategy. Purpose of the study. To perform a comparative analysis of refractive accuracy between paired and single intraocular lens (IOL) implantation in patients with high myopia. Materials and methods. A prospective comparative non-randomized clinical study was conducted from January 2018 to December 2025, including 97 patients (100 eyes) with high myopia and axial length ≥28.00 mm. Patients were divided into two groups: Group 1 (paired IOL, n=48) underwent implantation of two monofocal IOLs into the capsular bag with a target refraction of −1.00 to −1.50 D; Group 2 (single IOL, n=52) underwent implantation of a single monofocal IOL with a target refraction of −0.25 to −0.50 D. Biometry was performed using the IOLMaster 700, and IOL power calculation was based on the Barrett Universal II and Kane formulas. Results. Groups were comparable in preoperative parameters: age (33.47 ± 7.49 vs 34.82 ± 6.94 years, p=0.36), spherical equivalent (−16.22 ± 5.56 vs −15.84 ± 5.12 D, p=0.72), and axial length (29.17 ± 2.24 vs 28.92 ± 2.08 mm, p=0.58). At 5 years postoperatively, the mean residual spherical equivalent was −1.34 ± 1.20 D in the paired IOL group and −0.48 ± 0.86 D in the single IOL group. The mean absolute prediction error was 0.68 D in the paired IOL group and 0.42 D in the single IOL group (p=0.004). Refractive predictability within ±1.00 D was achieved in 76.3% (paired IOL) and 94.2% (single IOL) of eyes (p=0.01). The single IOL group demonstrated a statistically significant advantage in accuracy and reproducibility of outcomes. Conclusions. Paired IOL implantation may be considered an effective alternative to single IOL implantation in patients with high myopia when the calculated power of a single IOL falls outside the standard range of available models or requires lenses with extremely low or negative optical power, despite a statistically lower refractive predictability compared to single IOL implantation.
