Cardiorenometabolic syndrome (CRMS) is one of the most important challenges in modern medicine and is considered a leading cause of morbidity and mortality worldwide. Major randomized clinical trials conducted over the past five years — SELECT (2023), FLOW (2024), and STEP-HFpEF (2023) — have provided fundamentally new insights into the cardiovascular and renal protective mechanisms of GLP-1 receptor agonists (GLP-1 RAs), particularly semaglutide. The effects of GLP-1 RAs extend beyond glycemic control and involve a multifaceted biological network targeting endothelial dysfunction, oxidative stress, NF-κB–mediated inflammation, activation of the renin–angiotensin–aldosterone system (RAAS), intraglomerular hyperfiltration, and visceral adiposity. Current clinical and mechanistic evidence provides sufficient rationale to consider this drug class as one of the key pillars of future CRMS management.
