Post-osteomyelitic diaphyseal defects of the femur are among the most complex complications in traumatology and orthopedic practice, frequently leading to disability. Chronic osteomyelitis in this region is often characterized by significant angiotrophic disturbances, bone tissue deficiency, and technical limitations in the application of classical osteosynthesis and distraction osteogenesis methods. Therefore, the development of biologically grounded and technically accessible methods using external fixators to restore femoral integrity while maintaining its length and function is a critical task of modern reconstructive surgery. This study describes a technique for treating post-osteomyelitic diaphyseal femoral defects using autograft replacement, which ensures the preservation of limb length and reduces the risk of pathological complications.
