The review article presents an analysis of contemporary scientific publications devoted to the surgical treatment of focal periodontal lesions using tooth-preserving flap surgeries. Epidemiological aspects of periodontal diseases, classification and pathogenetic features of focal periodontitis, and the evolution of flap surgical techniques from the classical Cieszynski–Widman procedure to modern microsurgical approaches are considered. Particular attention is paid to the role of osteoplastic materials, guided tissue regeneration (GTR) membranes, biologically active growth factors (PRF, enamel matrix proteins), and combined protocols in improving the predictability of treatment outcomes. Criteria for selecting the technique depending on the morphology of the bone defect are analyzed, and directions for further research in tooth-preserving periodontal surgery are identified.
