Based on the results of clinical and instrumental examinations and analysis of case histories, the authors evaluate the problems of rhinogenic intracranial complications. The results of surgical treatment of nine patients aged 11 to 18 years with purulent diseases of the nose and paranasal sinuses are presented. Among the examined patients, frontal lobe abscess of the brain was diagnosed in two patients, epidural empyema in six patients (two of them were simultaneously diagnosed with sagittal sinus thrombosis, in one patient epidural empyema was complicated by diffuse meningitis. It was concluded that rhinoendoscopy, MSCT and MRI examination of the sinuses, brain and study of the composition of cerebrospinal fluid allows to verify the initial stage of rhinogenic intracranial complications. The development of rhinogenic intracranial complications requires urgent sanitizing surgery. According to the authors, it is necessary to perform minimally invasive interventions on the sinuses against the background of adequate antibacterial therapy. Radical interventions in the sinuses should be carried out when the condition of patients worsens against the background of adequate treatment, and in case of brain abscess, urgent surgical interventions both in the sinuses and in the brain. The presented system of complex emergency intensive care can improve treatment results and reduce Mortality in rhinogenous intracranial complications.
