This article presents the results of adapting the Filatov screen-based simulator (GEOTAR-Media) to the healthcare system of the Republic of Uzbekistan, as well as the development and pilot testing of a three-module training program based on the simulator. The adaptation process included translating the interface into Uzbek, modifying the clinical scenario library to reflect relevant disease categories, aligning diagnostic and treatment algorithms with national clinical protocols, and implementing technical improvements to the software. The training program comprises 36 sessions totaling 108 academic hours and includes 136 clinical scenarios distributed across three modules: basic, intermediate, and advanced. The program was pilot-tested with 285 third- to sixth-year medical students. In all three modules, task performance improved significantly from the first to the final session (p < 0.001). The greatest improvement in knowledge was observed in the basic module, amounting to 34.1 ± 7.8 points, or 80.6% (p < 0.001). The advanced module yielded the highest final score (79.6 ± 9.8) and the largest proportion of excellent grades (52.1%; p < 0.05). Positive student evaluations of the training accounted for 90.2% of responses. The adapted simulator received an average expert rating of 8.5 out of 10, with a high level of agreement among experts (Kendall’s W = 0.78; p < 0.001). The findings support the feasibility of integrating the adapted screen-based simulator and modular training program into undergraduate medical education in Uzbekistan.
