Relevance. Glaucoma remains one of the leading causes of irreversible blindness worldwide. The development of early diagnostic methods, particularly for patients with high myopia, is a pressing issue. Purpose of the study. To evaluate the diagnostic value of the «temporal raphe» sign in detecting structural and functional changes in patients with primary open-angle glaucoma. Materials and Methods. The study included 38 patients (62 eyes) diagnosed with primary openangle glaucoma. All patients underwent comprehensive ophthalmologic examination, including visual acuity testing, tonometry, gonioscopy, standard automated perimetry, and optical coherence tomography. The «temporal raphe» sign was assessed using the ganglion cell-inner plexiform layer (GCIPL) thickness map. Results. Statistically significant differences in morphometric and functional parameters were identified between eyes with positive and negative «temporal raphe» signs. The «temporal raphe» sign demonstrated high diagnostic value (AUROC 0.965), surpassing traditional indicators. Conclusion. The «temporal raphe» sign is a highly sensitive marker of early glaucomatous changes and may be effectively used in clinical practice to enhance early glaucoma diagnosis, especially in patients with high myopia.
