Relevance. The combination of endocrine ophthalmopathy (EO) with ocular hypertension (OHT) and glaucoma presents a diagnostically and therapeutically complex clinical scenario. In the absence of a unified management strategy, there is a high risk of delayed detection of glaucomatous changes and irreversible visual loss. Purpose of the study. To evaluate the clinical course of OHT and glaucoma in patients with EO and to develop an optimal management strategy based on clinical-functional and morphological parameters. Materials and methods. A total of 38 eyes of patients with EO and elevated intraocular pressure were examined. The assessment included tonometry, anterior segment OCT, analysis of Schlemm’s canal, retinal nerve fiber layer (RNFL) thickness measurement, and computer perimetry. Patient management was based on a developed clinical algorithm. Treatment effectiveness was evaluated by monitoring changes in Visual Field Index (VFI) and Mean Deviation (MD) over a two-year period. Results and conclusion. Complete stabilization of functional parameters was achieved in 100% of eyes with confirmed glaucoma and in 87.5% of eyes with OHT without signs of glaucoma. In 12.5% of cases, disease progression and transition to glaucomatous pathology were observed, requiring the initiation of therapy. An individualized approach with regular structural and functional monitoring enables effective control of OHT progression and early detection of glaucomatous damage in patients with EO.
