Relevance. Post-occlusion ischemic maculopathy (IM) remains an underexplored complication of retinal vein occlusion (RVO), despite its significant impact on central visual function. Diagnosis of IM is challenging due to the lack of standardized assessment criteria and limited early visualization methods. Objective. To assess the prevalence of IM and its effect on best-corrected visual acuity (BCVA) in patients with post-occlusion retinopathy using optical coherence tomography angiography (OCTA). Materials and Methods. Forty patients (42 eyes) with confirmed RVO (duration 21 days to 4 months) were examined. All underwent standard ophthalmologic evaluation and OCTA of the macular region (Solix Optovue, 3×3 mm² and 6×6 mm²). The following parameters were assessed: foveal avascular zone (FAZ) metrics, capillary perfusion density, and vessel density. The severity of IM was graded according to the ETDRS classification. Statistical analysis was performed using the Mann–Whitney test and Pearson’s correlation coefficient. Results. IM was detected in 100% of eyes. IM levels 1 and 2 accounted for 85.7% of cases and were associated with relatively preserved FAZ structure and BCVA. In levels 3 and above, FAZ enlargement, reduced circularity, and marked decline in BCVA were observed. In IM level 8, OCTA quantification was not possible due to severe structural distortion. Conclusion. OCTA provides an objective assessment of IM severity and its impact on vision. The FAZ circularity index may serve as a sensitive marker for risk stratification of visual acuity loss in patients with post-occlusion retinopathy.
