Relevance. Involutional malpositions of the lower eyelid, including entropion, are common age-related conditions that cause not only cosmetic but also significant functional disturbances. Malposition of the eyelid leads to tear film instability, chronic ocular surface irritation, increased risk of corneal damage, and the development of dry eye symptoms. Purpose. To conduct a comprehensive assessment of ocular surface parameters and tear film stability in patients with involutional lower eyelid malpositions before and after surgical correction using the tunnel canthoplasty technique. Materials and Methods. The study included 41 patients (41 eyes) with a confirmed diagnosis of involutional lower eyelid entropion. All patients underwent surgical correction using tunnel canthoplasty. Examinations were performed preoperatively, as well as at 2 weeks, 3 months, and 6 months postoperatively. The assessment included ocular surface evaluation (fluorescein staining using the Oxford scale, LIPCOF test), tear film stability (Norn’s test, Schirmer I and II tests), and subjective symptom severity (OSDI questionnaire). Results. Significant improvement was observed in all measured parameters during both early and late postoperative periods: reduction in the degree of conjunctivochalasis, increased tear film stability (prolongation of tear break-up time), normalization of tear production, and a marked decrease in subjective symptoms of irritation and dryness based on OSDI scores. The tunnel canthoplasty technique demonstrated high clinical efficacy and stability of outcomes without serious complications. Conclusion. Tunnel canthoplasty is an effective and safe surgical method for correcting involutional entropion, providing restoration of eyelid anatomy, improved tear film stability, and ocular surface condition with no major complications.
