This study investigated the prognostic significance of BDNF and IL-6 biomarkers and neuropsychological indicators in predicting speech recovery in patients with post-stroke aphasia. A total of 110 patients with post-stroke aphasia underwent comprehensive clinical, neurological, neuropsychological, and laboratory assessments. Aphasia severity was evaluated using the Boston Diagnostic Aphasia Examination and the Western Aphasia Battery. Serum BDNF and IL-6 levels were measured using enzyme-linked immunosorbent assay. The collected data were analyzed using correlation and regression methods. The results demonstrated a strong positive correlation between BDNF levels and speech recovery scores (r = 0.83, p < 0.001) and a strong negative correlation between IL-6 levels and speech outcomes (r = –0.91, p < 0.001). Integrated assessment of biomarkers and neuropsychological parameters enables early prediction of rehabilitation outcomes and supports the development of individualized rehabilitation strategies.
