Acute cholecystitis (AC) is one of the most common surgical diseases in elderly patients, characterized by high mortality and complication rate. This article deals with the peculiarities of clinical course, diagnosis and treatment of AC in elderly patients. It is shown that in this category of patients the disease often has a subtle symptomatology, which complicates timely diagnosis. Modern diagnostic methods, including ultrasound, computed tomography and laboratory markers of inflammation are discussed. Treatment tactics including drug therapy, laparoscopic cholecystectomy, and percutaneous cholecystostomy in debilitated patients are reviewed. The importance of an individualized approach to the choice of treatment method is emphasized to reduce surgical risks and improve prognosis.
