Elastography is a modern ultrasonic research method based on assessing the mechanical properties of tissues using ultrasonic signals. The methodology allows not only for obtaining qualitative images but also for quantitative measurements reflecting the degree of rigidity of the examined tissues, which is an important diagnostic indicator in identifying pathological changes. Thanks to the application of advanced technologies, elastography is becoming an indispensable tool in the arsenal of modern visual diagnostics, ensuring high informativeness and objectivity of the obtained data. Endoscopic ultrasound examination (EUS) occupies a key place in the diagnosis and treatment of a wide range of pathologies covering various anatomical areas, such as the abdominal cavity, retroperitoneal space, and mediastinum. This method is characterized by high sensitivity and specificity, which allows it to be successfully used in the diagnosis of both invasive and non-invasive pathological processes. In recent years, the integration of elastographic methods into EUS practice has significantly expanded the diagnostic capabilities of this approach, allowing for obtaining additional information about the characteristics of tumor and inflammatory processes. The use of EUS elastography in clinical practice contributes to a comprehensive assessment of the elasticity of tumor formations, as well as pathologically altered areas of tissues located near the digestive tract. The method is of particular importance in the study of structures access to which is difficult when using traditional transdermal ultrasound sensors. For example, pancreatic tumors and lymph nodes located in the mediastinum or abdominal cavity are often objects that require additional diagnostic methods for evaluation. Thus, EUS elastography not only increases the accuracy of visualization but also significantly increases the diagnostic effectiveness of the study due to the ability to determine the degree of tissue rigidity, which is an important criterion for assessing the malignancy or benignity of the pathological process. The results of numerous previous studies demonstrate that the use of EUS elastography has significant advantages in the differential diagnosis of lymph nodes. The application of this method allows not only to identify pathologically altered nodes but also to conduct a detailed comparison of their structural characteristics with normal tissues, which significantly facilitates the determination of the nature of the pathological process. In turn, assessing the elasticity of pancreatic and other gastrointestinal tract tumor formations provides an opportunity to accurately locate and determine the nature of the identified changes. This, in turn, is an important factor in choosing further treatment tactics and managing patients. It should be emphasized that EUS elastography is not intended to completely replace biopsy, which remains the gold standard in morphological diagnostics. However, this method can serve as an invaluable complement to traditional diagnostic techniques, significantly improving the accuracy of EUS-guided fine-needle aspiration biopsy (EUS-FNAB). The ability to preliminarily identify areas with the highest suspicion based on stiffness indicators allows specialists to conduct biopsy sampling more precisely, which significantly improves diagnostic accuracy and reduces the likelihood of false-positive or false-negative results. In cases where EUS-FNAB results are negative or inconclusive, elastography can serve as an important guide for subsequent clinical monitoring and decision-making regarding the need for repeated examination or invasive intervention. Moreover, current research highlights the promising use of EUS elastography for assessing and monitoring the dynamics of pathological processes. For instance, in the context of monitoring patients with chronic pancreatitis or precancerous conditions of the pancreas, regular elastographic studies can detect microscopic tissue changes long before the appearance of obvious clinical symptoms. This opens up new opportunities for early diagnosis and prevention of malignant processes, as well as for evaluating the effectiveness of ongoing therapy. Additionally, the use of elastography in combination with other modern imaging methods, such as contrast-enhanced ultrasound, allows for a comprehensive assessment of organ morphology and functional state, which is a significant advantage in modern medicine. This article examines in detail the current knowledge of EUS elastography, focusing on its technical features, such as the principles of elastographic image formation, research methodology, and the specifics of processing and interpreting the obtained data. Special attention is paid to the comparative analysis of the diagnostic effectiveness of EUS elastography in the differential diagnosis between benign and malignant solid tumors of the pancreas, as well as lymph nodes. The article evaluates the method's capabilities in differentiating normal pancreatic tissue from areas affected by chronic pancreatitis, which is particularly important in the context of early diagnosis and treatment strategy determination. New indications for the use of EUS elastography are also discussed, including the expansion of the range of diagnosed pathologies through examination of organs such as the liver, kidneys, bile ducts, adrenal glands, and other parts of the gastrointestinal tract. The prospects for the development of the method are related to the improvement of software for processing elastographic images, the implementation of artificial intelligence algorithms for automating data interpretation, as well as conducting multicenter clinical trials aimed at standardizing research protocols. These research directions have the potential to significantly increase diagnostic accuracy, improve clinical practice, and shorten the time to diagnosis, which is especially important for patients with oncological and inflammatory diseases. Thus, the presented review of the current aspects of using EUS elastography demonstrates the high clinical significance of this method, its potential for improving the quality of diagnostics and the effectiveness of treatment measures. The integrated use of EUS elastography in combination with traditional research methods significantly expands the diagnostic arsenal and optimizes the choice of subsequent treatment strategies, which is important for improving the prognosis of patients with various pathologies. The presented data indicate the need for further research in this area to improve visualization methods and enhance their clinical applicability.
