Relevance. Dry eye syndrome (DES) is a multifactorial disease of the ocular surface, often exacerbated by environmental and systemic conditions. Atrophic rhinitis, a chronic condition of the nasal mucosa characterized by tissue thinning and dysfunction, may influence the lacrimal system due to the anatomical and physiological connection between the nasal and ocular cavities. However, the relationship between DES and atrophic rhinitis remains underexplored. Purpose of the study. To evaluate the ocular surface status in patients with atrophic rhinitis and investigate its association with the severity of dry eye symptoms and signs. Material and methods. A total of 86 patients (172 eyes) were examined and categorized into two groups: the main group included 42 patients with clinically confirmed atrophic rhinitis, and the control group comprised 44 patients with DES without rhinitis. All participants underwent a comprehensive ophthalmic evaluation, including Norn’s test for tear film breakup time (TBUT), Schirmer’s test, the Ocular Surface Disease Index (OSDI) questionnaire, meibography, and optical coherence tomography (OCT) meniscometry. Results. TBUT was significantly shorter in patients with atrophic rhinitis compared to the control group (5.6 sec vs. 6.6 sec; p < 0.001), indicating greater tear film instability. The Schirmer’s test did not show significant differences (p > 0.05), suggesting comparable tear production. OSDI scores were higher in the main group, reflecting more pronounced subjective symptoms. Meibography changes were more marked in the main group but lacked statistical significance. OCT meniscometry revealed reduced tear meniscus height and wettability angle in patients with rhinitis. Conclusion. Patients with atrophic rhinitis exhibit more severe clinical manifestations of DES, predominantly due to tear film instability rather than aqueous deficiency. These findings underscore the importance of an interdisciplinary approach to managing such patients, considering the shared pathophysiological mechanisms between nasal and ocular surface disorders.
