The article is devoted to the principles and methods of treatment in the acute period of cardioembolic stroke. Basic (undifferentiated) and specific (differentiated) therapy, taking into account the etiology and pathogenesis of this subtype of ischemic stroke, as well as surgical and endovascular methods alternative to medical treatment methods, are described. Indications, criteria for the selection of patients who are shown intravenous thrombolysis, management tactics of these patients, complications are displayed. The comparative characteristics of direct and indirect anticoagulants, inhibitors of the Xa factor of the blood coagulation system, platelet antiplatelet agents used to prevent repeated cardioembolism are presented. The schemes of prescribing drugs used in cardioembolic stroke for the purpose of neuroprotection are described.
