Relevance.The diagnosis of primary hyperparathyroidism (PHPT) was made on the basis of examination of total calcium and inorganic phosphorus content in blood and in daily urine, ionised calcium in blood, exercise tests with calcium and sodium chloride, parathyroid hormone (PTH), calcitonin (CT), vitamin D and cyclic 3,5 β-adenosine monophosphate (cAMP) content in serum. Purpose of the study: to evaluate the degree of bone demineralisation depending on the form of primary hyperparathyroidism (PHPT) and urolithiasis. Materials and Methods: A total of 2100 children aged 1 to 15 years with urolithiasis were examined, of whom 52 (2.5%) children were diagnosed with primary hyperparathyroidism (PHPT). The renal form (RF) of PHPT was 29 (55.7%), and the mixed form (MF) (renal and bone lesions) of PHPT was in 23 (44.2%) patients. To detect bone lesions, we investigated alkaline phosphatase (ALP) activity and X-ray densitometry of hand bones and ultrasonic bone osteometry. Results: High values of ALP and low values of echo-osteometry were observed in children with renal dysfunction and had a direct correlation with the number and size of formed stones and bone osteoporosis. Discussion: ALP and bone echoostometry provide an opportunity to assess the severity of osteoporozis and, accordingly, to differentiate PHPT according to the forms (renal and mixed) and severity of the disease.
