OPTIMIZATION OF SURGICAL TREATMENT OF POSTOPERATIVE VENTRAL HERNIAS BASED ON LAPAROSCOPIC PROSTHETIC METHODS

Objective. To improve the effectiveness of surgical treatment of postoperative ventral hernias by optimizing the choice of hernioplasty technique based on the ratio of hernia volume to the abdominal cavity, as well as developing a surgical strategy algorithm aimed at reducing the incidence of postoperative complications and recurrence. Materials and Methods. The study is based on a clinical and laboratory examination of 109 patients with postoperative ventral hernias who underwent surgery in the surgical department of the multidisciplinary clinic of Samarkand State Medical University between 2015 and 2024. All patients were admitted for elective surgery. Depending on the treatment strategy, patients were divided into 2 groups. Results. A comparative analysis of the use of standard and modified trocars with air chambers was conducted. Long-term outcomes were analyzed for 84.4% of patients. Hernia recurrence was detected in 7.6% of patients: 11.3% in the comparison group and 2.6% in the study group. Conclusion. The laparoscopic hernioplasty technique demonstrated advantages over the open technique, including shorter operative time, reduced postoperative pain, earlier patient mobilization, and a reduced complication rate. Implementation of the proposed technical solutions contributes to improved surgical outcomes for small and medium-sized ventral hernias.
