THE IMPACT OF CONCOMITANT SURGICAL PATHOLOGY ON THE OUTCOMES OF SIMULTANEOUS OPERATIONS IN PATIENTS WITH VENTRAL HERNIAS AND MORBID OBESITY

Annotatsiya

The outcomes of surgical treatment were analyzed in 156 patients with morbid obesity and ventral hernias. The main group consisted of 102 patients who underwent simultaneous surgical procedures with one-stage correction of concomitant surgical pathology. Intraoperative parameters, postoperative complication rates, length of hospital stay, long-term outcomes, and quality of life assessed using the SF-36 questionnaire were evaluated. It was established that all patients in the main group had concomitant surgical pathology, most commonly represented by adhesive disease of the abdominal cavity, cholelithiasis, and pathology of the pelvic organs. Adhesive disease of the abdominal cavity had the greatest impact on operative time and complication rates. However, the use of a simultaneous surgical approach did not lead to a statistically significant increase in wound-related or extra-abdominal complications. In the long-term follow-up, a reduction in the recurrence rate of ventral hernias and a significant improvement in quality-of-life indicators were observed. The presence of concomitant surgical pathology is not a contraindication to performing simultaneous operations in patients with morbid obesity. A rationally planned one-stage correction of combined diseases ensures stable clinical outcomes and improved quality of life.
