Relevance. The current stage of development of medicine indicates that the number of patients with chronic heart failure (CHF) is increasing, the risk of developing a decompensated stage of CHF is high, and the number of re-hospitalizations is increasing. The deterioration of the condition of patients with CHF requires a change in the tactics of treatment and rehabilitation of patients at the outpatient stage. Target. Improving the organization of medical care for patients with chronic heart failure by opening a Patient Monitoring Center at the outpatient stage. Materials and methods. 300 patients with a diagnosis of hypertension, coronary artery disease, type 2 diabetes, post-infarction cardiosclerosis, complicated by CHF were included. 150 patients were observed in an outpatient clinic, 150 patients were observed at a multidisciplinary city clinic where a CHF center was opened. Results. After a year of observation, cardiovascular mortality was statistically significantly lower in group 1 - 2.7% compared to group 2 - 5.0% (p<0.001). When analyzing CVD mortality between groups, statistically significant differences were found in the form of a 9-fold increase in the risk of CVD mortality in group 2 compared to group 1. This fact indicates that in group 1 effective measures were taken in a timely manner to prevent cardiovascular death. Conclusion. Changing the organization of medical care for patients with CHF will lead to an improvement in the life expectancy of patients and will significantly increase the rehabilitation potential, which is economically beneficial by reducing the number of patients with CHF and the frequency of hospitalizations for repeated decompensation of CHF.
