The article presents the neuroimaging characteristics of non-functioning giant pituitary adenomas based on an analysis of magnetic resonance imaging (MRI) data. The study aimed to assess the mechanical factors influencing tumor growth on surrounding anatomical structures. It involved 22 patients, with an equal number of men and women, and an average age of 45.7 and 48.5 years, respectively. The analysis examined various tumor growth directions and their correlation with clinical manifestations. The most common disorder associated with endo-suprasellar adenoma growth was visual field defects (chiasmal syndrome), while retro-sellar growth resulted in cranial nerve involvement and pyramidal symptoms. The features of lateral, infra-, and antero-sellar tumor growth and their impact on patients' neurological and ophthalmological conditions are also described. It was noted that total tumor growth caused the most severe neuroendocrine, ophthalmological, and brainstem disorders. The authors highlight that the invasive nature of giant pituitary adenoma growth significantly limits the radicality of surgical treatment and increases the risk of recurrence.
