Electrical and structural cardiac remodeling in young football players: association between ECG-reported LVH and echocardiographic LVMI

Objective – to assess electrical and structural cardiac remodeling in young male football players and determine the association between left ventricular hypertrophy (LVH) reported in the electrocardiogram (ECG) protocol and echocardiographic left ventricular mass index (LVMI). This retrospective cross-sectional analysis included data from 23 football players who underwent sports-medical screening between March and June 2026. Echocardiographic data were available for all 23 athletes and primary ECG data for 22. Athletes with and without ECG-reported LVH were compared using the Mann–Whitney U test, and associations between morphofunctional variables were assessed using Spearman rank correlation. Sinus bradycardia/bradyarrhythmia was reported in 18/22 (81.8%) and ECG-LVH in 16/22 (72.7%). Mean LVMI was 88.70±9.46 g/m² and ejection fraction was 61.22±2.95%. LVMI was 90.63±9.67 g/m² in the ECG-LVH group and 85.00±8.32 g/m² in athletes without ECG-LVH; the difference was not statistically significant (U=60.0; p=0.396). Left ventricular mass correlated positively with end-diastolic volume (ρ=0.597; p=0.003) and stroke volume (ρ=0.515; p=0.012). The findings indicate that ECG-reported LVH in young football players should not be interpreted as a direct equivalent of anatomical hypertrophy and support integrated ECG–echocardiographic assessment.
