A DEVICE FOR ACETABULAR REAMING IN HIP ARTHROPLASTY

Introduction. Preparing the acetabulum is one of the most technically challenging stages of total hip arthro­plasty. The use of standard instruments does not always ensure the uniform removal of marginal osteophytes and the formation of a stable support surface for the acetabular component. Objective. To develop and clinically test a device for preparing the acetabulum during hip arthroplasty, present its design features and surgical technique, and evaluate the initial results of its clinical use. Materials and Methods. A prospective clinical trial of the developed device was performed on 10 patients who underwent primary (n=8) or revision (n=2) total hip arthroplasty. We assessed the technical feasibility of using the device, the quality of acetabular preparation, the ability to implant the acetabular component in the planned position, intraoperative complications, and the results of clinical and radiological follow-up for 6 months. Results. In all cases, the device was successfully used without any technical failures or the need to switch to conventional reaming methods. Complete removal of marginal osteophytes, uniform shaping of the bearing surface, and implantation of the acetabular component in the planned position were achieved in all patients. No intraoperative complications associated with the use of the device were recorded. Follow-up radiographs confirmed the stable position of the endoprosthesis components, and no signs of implant migration or revision surgery were noted during the six-month follow-up period. Conclusion. The developed device provides for standardized and reproducible reaming of the acetabulum, facilitates the removal of marginal osteophytes, and creates a smooth bearing surface for the installation of the acetabular component. The initial results of clinical trials have confirmed the technical reliability, safety, and practical applicability of the proposed design, which indicates the promise of its further clinical study.
