This study evaluated the association between carriage of the CYP2E1 c2 allele and nutritional risk and quality of life in patients with alcohol-related liver cirrhosis. The analysis included 60 patients: 39 with the c1/c1 genotype and 21 carriers of the c1/c2+c2/c2 genotypes. c2 carriers had significantly lower body mass index and mid-arm circumference and a higher NRS-2002 score. Nutritional deficiency was identified in 90.5% of c2 carriers compared with 59.0% of patients with the c1/c1 genotype, yielding an odds ratio of 6.61. The SF-36 physical and mental component scores were lower by 10.8 and 11.4 points, respectively, among c2 carriers. c2 carriage correlated positively with NRS-2002 and MELD/MELD-Na and negatively with serum albumin and the SF-36 physical component score. These findings support early nutritional screening and systematic quality-of-life assessment in c2 carriers with alcohol-related liver cirrhosis. Purpose – to assess the association of CYP2E1 c2 allele carriage with anthropometric parameters, nutritional risk, and quality of life in patients with alcohol-related liver cirrhosis. Material and methods – The study included 60 patients with alcohol-related liver cirrhosis: 39 with the c1/c1 genotype and 21 carriers of c1/c2+c2/c2. Nutritional status was assessed using body mass index, mid-arm circumference, the NRS-2002 scale, and the frequency of nutritional deficiency. Quality of life was evaluated using the physical and mental component scores of the SF-36. Between-group comparisons, the χ² test, Spearman correlation, and odds ratios with 95% confidence intervals were applied. Results – Among c2 carriers, body mass index was 19.1±3.2 versus 21.6±3.4 kg/m², mid-arm circumference was 21.8±3.2 versus 24.9±3.5 cm, and NRS-2002 was 4.5±1.2 versus 3.4±1.0 points. Nutritional deficiency occurred in 90.5% versus 59.0% of patients (OR=6.61; 95% CI: 1.35–32.43; p=0.011). SF-36 PCS and MCS were lower by 10.8 and 11.4 points, respectively, in c2 carriers. c2 carriage correlated with MELD/MELD-Na (rₛ=0.49), NRS-2002 (rₛ=0.47), albumin (rₛ=−0.44), and SF-36 PCS (rₛ=−0.51). Conclusion – CYP2E1 c2 allele carriage in alcohol-related liver cirrhosis is associated with more pronounced nutritional deficiency and lower physical and mental quality-of-life scores. This genetic marker may serve as an additional signal for early nutritional screening and intensified follow-up; however, the observed associations require confirmation in larger independent cohorts.
