Acute appendicitis in patients with liver cirrhosis: epidemiology, pathogenesis and clinical features

Annotatsiya

Acute appendicitis in patients with liver cirrhosis represents a challenging clinical and surgical condition characterized by atypical presentation, delayed diagnosis, high complication rates, and increased mortality. Liver cirrhosis significantly alters the course of acute abdominal inflammatory processes due to portal hypertension, coagulation disorders, cirrhosis-associated immune dysfunction, and ascites. This narrative review summarizes current evidence on the epidemiology of acute appendicitis in cirrhotic patients, key pathophysiological mechanisms underlying complicated disease course, and specific clinical features affecting diagnosis and outcomes. Particular emphasis is placed on the prognostic value of Child–Pugh and MELD scores in surgical risk stratification, as well as on factors contributing to poor postoperative outcomes. The reviewed data highlight the need for tailored diagnostic and therapeutic strategies in this high-risk patient population.
