The article discusses the spread of smallpox in the Fergana region in the late XIX - early XX centuries, its consequences and the problems of vaccination against the disease, the policy of the Russian Empire in this regard. No disease in Fergana province was as fatal as smallpox. The catastrophe occurred every year in both the settled and nomadic populations in different parts of the province, leaving behind a large number of dead, disabled and incurable patients. Also, a large proportion of child deaths were due to smallpox. Vaccination processes have been slow due to a misconception among the local population about the smallpox vaccine. The Russian government was reluctant to allocate large sums of money and effort in this regard.
