Актуальность. Синдром поликистозных яичников (СПКЯ) является одним из наиболее распространённых эндокринных расстройств у женщин репродуктивного возраста и характеризуется выраженной клинико-метаболической гетерогенностью. Фенотипическая стратификация позволяет уточнить клинический прогноз и индивидуализировать подходы к терапии. Цель. Оценить клинико-гормональные и метаболические особенности различных фенотипов СПКЯ и определить информативные маркёры для ранней диагностики заболевания. Материалы и методы. Обследованы 145 пациенток с СПКЯ, разделённые на четыре фенотипа по критериям Rotterdam (2003), и 22 женщины контрольной группы. Оценивались клинические, ультразвуковые, гормональные и метаболические показатели. Статистический анализ включал регрессионные модели, ROC-анализ и нейросетевые алгоритмы. Результаты. Наиболее неблагоприятный профиль выявлен у пациенток с фенотипом А (ожирение, инсулинорезистентность, дислипидемия, выраженная гиперандрогения). Фенотип D характеризовался минимальными нарушениями и приближался к контрольной группе. Ключевые маркёры ранней диагностики СПКЯ: длительность менструального цикла, уровень тестостерона, индекс массы тела, объём яичников и количество антральных фолликулов. Заключение. Фенотипическая стратификация и использование диагностически значимых маркёров позволяют повысить точность ранней диагностики СПКЯ и сформировать персонализированные стратегии ведения пациенток.
