Arterial hypertension (AH) remains the leading modifiable risk factor for cardiovascular diseases. The aim of the study is to identify gender characteristics of hypertension phenotypes in the working-age population and to develop a screening algorithm for primary healthcare. The cross-sectional study included 1,798 patients with newly detected hypertension (704 men, 1094 women) from 4 clinics in the Navoi region (Uzbekistan). Analysis of clinical-anamnestic, anthropometric, laboratory data, and quality of life was conducted. A screening algorithm questionnaire with weighting coefficients has been developed. "Rejuvenation" of hypertension was identified: 30.1% of patients were aged 25-44. Men were characterized by behavioral risk factors (31.5% smoking, 36.2% alcohol consumption), while women were characterized by metabolic risk factors (42.5% obesity, 68.6% dyslipidemia, 38.8% chronic kidney disease). Depression (HADS ≥8) is associated with obesity (OR=2.22; 95% CI 1.78–2.77) and dyslipidemia. An 11-factor screening algorithm has been developed with risk thresholds: low (0–4), moderate (5–9), high (10–14), and very high (≥15). Two gender phenotypes of hypertension were identified: "behavioral" (men) and "metabolic-psycho-emotional" (women). The proposed screening algorithm is adapted for primary care conditions and requires prospective validation.
