For many years, plastic surgeons have experimented with various materials in an attempt to enhance and correct deformities of the facial soft tissues. Among all existing methods, autologous fat grafting (lipofilling) stands out for several notable advantages: it leaves no scars, is technically straightforward to perform, and is generally well tolerated by patients. Today, lipofilling has become one of the most popular and widely used procedures in facial plastic surgery. However, the effectiveness of this technique is still limited by two main challenges: insufficient vascularization of the graft after transplantation and the low viability and proliferative capacity of adipocytes. This article reviews the historical development of facial soft tissue augmentation techniques and highlights the role of adipose-derived stem cells in the modern clinical management of facial soft tissue deformities.
