The study evaluated the effectiveness of combined treatment for differentiated thyroid cancer (DTC) considering morphological and molecular-genetic characteristics. A total of 300 patients treated at the Tashkent Branch of the Republican Specialized Scientific and Practical Medical Center of Oncology and Radiology (2014–2024) were included. The use of an improved algorithm (ultrasound, FNAB, molecular testing, and personalized surgery) resulted in higher two-year recurrence-free survival (78.1% vs. 66.5%; p=0.033). Unfavorable prognostic mutations included TERT, RAS, PAX8-PPAR-γ, and BRAF-V600E. Extended thyroidectomy with lymph node dissection reduced recurrence risk. Integration of molecular profiling improved survival and reduced treatment costs.
