The results of the treatment of 144 patients with obstructive jaundice, which arose as a complication of gallbladder disease, were presented. Factor analysis of mechanical jaundice showed that the main cause of death is cholangiogenic liver abscess and biliary sepsis. Fatal outcomes (14.8%) and purulent-septic complications (44.4%) were observed mainly after emergency surgical interventions performed as a result of acute purulent cholangitis accompanied by acute destructive cholecystitis and peritonitis. Taking into account the severity of mechanical jaundice, the use of primary decompressive interventions on the bile ducts to relieve cholestasis and cases of purulent poisoning can improve the results of radical operations in ham. At the same time, 81.8% of patients with acute purulent cholangitis were able to perform diapeutic and endoscopic transduodenal interventions in patients with severe nausea, 61.6% - with moderate weight and 24.1% - with mild.
